The player takes on the role of Major Nechaev, an operative with the callsign "P-3," assigned to investigate a catastrophe at the "Facility 3826" research complex. However, a simple mission quickly turns into a frantic fight for survival as the world is overrun by deranged machines, terrifying mutants, and deadly secrets. Inspired by Soviet retrofuturistic aesthetics, Atomic Heart combines dynamic combat, a deep RPG system, and a dark atmosphere that makes players question the boundaries of science and morality.

The world of Atomic Heart expands with three unique underwater DLCs, each uncovering new secrets of the sunken Soviet utopia. The first DLC, "Depths of Madness", takes players into contaminated laboratories where a biomechanical plague has transformed humans and robots into terrifying abominations. Abandoned corridors hide archives shedding light on the catastrophe, while mutated creatures won’t let anyone escape easily. The next expansion, "Lost Echoes", delves deeper into underwater ruins, introducing a new class of enemies—cybernetic "drowned ones"—controlled by a collective AI. By solving ancient mysteries and discovering unique weapons adapted for the crushing depths, players get closer to uncovering the true purpose of the underwater complex. The final DLC, "The Last Bastion", serves as the culmination of the underwater saga. In bottomless shafts and ancient facilities guarded by titanic sentinels, a decisive battle unfolds.
